Help Bring Grandma Home

I am raising money because my family and I have made the decision to provide In-Home Care for our beloved grandmother. Currently she is in a nursing home and has been since March. Unfortunately grandma is suffering from a combination of medical ailments to include Alzheimer's disease, vision loss, loss of mobility, diabetes and high blood pressure. As a result of the Alzheimer's disease she is dealing with confusion and fear is setting in while living in the nursing home. I have always been incredibly close to both of my grandparents and I need and want to be with her and be there for her during this extremely difficult time. We don't know how much time she has left with us, but everyday for the rest of her life should be showered with love, understanding and she should never feel scared or alone. Now with the discharge process in motion and nearly complete I have a few request to help bring her home to us. I need help relocating her from NJ to NC. In addition to travel cost, there will be items that are necessary for her smooth transition and optimal health. She needs a hospital bed and hoyer lift, a recliner and other items that we may not foresee. Although Medicare/Medicaid will cover some of these items, because she is relocating to another state I will not be able to get prescriptions for these items until she is physically in NC and sees the Dr. here. This means that her medical equipment will have to be paid for out of pocket or rented until medicare/medicaid is able to cover them. We are providing for her all we can, but we do need help. I know that this is very short notice and money is tight for everyone, but any amount will be helpful and very much appreciated. Thank you all!
