Please help me!

My name is Steve Glover.  I'm 66 years old and live in New York City. I used to work for the federal government. I recently was diagnosed with congestive heart failure, pulmonary hypertension, 3rd stage renal failure, and portal hypertension (liver  disease). The liver disease is the worst.  I have ascites which causes me to leak from the abdomen.  It requires an ostomy bag to collect the fluid.  I still get soaking wet, ruining my furniture.  To make matters worse, I have an inoperable hernia the size of a cantaloupe on my lower abdomen.  There are sores on it that never heal,  I have a nurse come to change the bandages twice a week. I can't do it myself. On top of that I have severe peripheral neuropathy which causes my feet to contract and is excruciatingly painful. I sometimes can't walk.  I also get gout since my medication raises my uric acid level.  I may also have to go off my liver medication due to a creatinine level of 2.0, which could cause my kidneys to fail.

The insurance money from my mother's death is about to run out so my food budget will be cut in half, which is difficult since there wasn't much there anyway.  Especially since I'm on a low sodium diet due to fluid retention. I can't use food banks or dollar stores since their food is processed and high in sodium. I beginning to feel quite desperate about my financial situation. I need help!
